Imagine having the same round-the-clock care you’d expect in hospital, but from the comfort of your home. Hospital Care at Home combines in-home and virtual visits, giving you instant access to medical experts who can respond to your needs 24/7, just like a traditional hospital setting.
We provide technology to connect you to your care team at all times. This includes a phone, a remote vital sign monitoring device, a personal emergency response bracelet, a backup power supply, and a tablet for video visits.

ChristianaCare was named 2023 Program of the Year by Hospital at Home Users Group. Members of this group are health systems across the United States and Canada providing acute care to patients at home. They work together to share best practices and expand the reach of their hospital at home programs.
Hospital Care at Home is committed to the highest standards of care, reducing hospital readmissions, and improving patient outcomes. In August 2024, ChristianaCare’s Clinical Effectiveness Committee recognized our program for 1 year of “zero harm” for preventable hospital infections.

During your stay in the program, you will be provided with a tablet for video visits and a phone that connects you directly with your care team. In addition, you will also be provided with a personal emergency response bracelet, vital signs monitoring devices and a backup power supply. All technology is delivered and set up for you in your home.
The length of your stay in the Hospital Care at Home program is determined by you and your care team. Once you have reached your care goals, you will be discharged from the program. Our care team will make sure all your needs are met upon discharge. We will remove our technology (tablet, telephone, vitals sign monitoring equipment, and backup power supply). We will also follow up with you post-discharge to check on you and make sure you have everything you need to continue achieving your health and wellness goals.
