Blood Pressure Management

A simple solution for controlling your high blood pressure. Reach out to your Primary Care Provider for a referral to enroll.

Hypertension, or high blood pressure, can increase the risk of heart problems if undiagnosed. This is especially true with uncontrolled hypertension, when a blood pressure reading is over 130/80. If you know or suspect that you have it, your provider may be able to refer you to our Remote Patient Monitoring (RPM) program to help you better manage your condition.

How it works

  1. Enroll: On referral, we’ll set you up with a care team which will include a health coach and the providers supporting your care. They will send you a blood pressure cuff that sends your readings remotely for your provider to review — no need for Wi-Fi or Bluetooth. 
  2. Ongoing Support: Your care team is with you every step of the way, answering your questions and helping with things like prescription medicines and lab coordination.
  3. Covered by insurance or copay: We bill patient fees toy our insurance. This service is covered by most insurance companies. you may have a copay or deductible. Please check with your insurance provider if you have any questions.

 We partner with StoryHealth to bring you advanced virtual care for high blood pressure. After you enroll, your provider uses data from the StoryHealth to create a personalized care plan. With continuous data collection and feedback from you and your health coach, your provider will safely adjust medications based on daily blood pressure readings, symptom surveys, and your medical history.

No, all communication with your health coach is done over SMS text.

After enrolling, your health coach will send you a blood pressure cuff. This device fits on your upper arm and connects to your cell phone, automatically sending readings to your provider through the StoryHealth platform.

In most cases, the RPM program is covered by your insurance (with the exception of Cigna). We bill patient fees for service participation directly to your insurance provider, which includes the cost of the blood pressure cuff. You may have a copay or deductible agreement in your plan, so please ask your provider if you are unsure.

Typically, high blood pressure patients are enrolled for 3-4 months, depending on your individual needs.

If you have hypertension and meet these criteria, you can enroll with a referral from your provider:

  • Your blood pressure is greater than 130/80 
  • You are 18 years or older 
  • You or your caregiver has a smartphone
  • You live in a private residence.

Unfortunately, you are not eligible if:

  • You are in palliative care or a hospice 
  • You are pregnant, lactating, or breastfeeding 
  • You have a significant behavioral or psychiatric disorder 
  • Your upper arm circumference exceeds 42 cm (the cuff won't fit) 
  • You do not have a smart phone 
  • If you have chronic kidney disease (CKD), please talk to your provider or a Nephrology Consultant to check if RPM is suitable for you.
